This Assumption of Risk, Waver of Liability, Arbitration and Indemnification Agreement (“Agreement”) is dated as of the Effective Date, which is the date appearing on the signature page, between Family Adventures Milltown LLC d/b/a Urban Air Trampoline & Adventure Park Milltown (“Urban Air”) and the undersigned in his/her own capacity (“Adult Participant”) and if any minor(s) is/are named in the signature block below (collectively “Child Participant”, whether one or more) on behalf of, and as the parent or legal guardian for such Child Participant(s) (all parties collectively, “Participant”). As valid consideration for entry into the premises located at 300-396 Ryders Lane, Milltown, NJ 08850 (“Premises”) and participation in the Activities (as defined below), the undersigned agree as follows:
