This Student Enrollment Contract (the “Agreement”) is made between Liberty Eagle Academy, LLC and the person(s) whose signature(s) appear on this Agreement on the line(s) below identified as “Signature of Parent(s)” (all of whom are collectively referred to in this Agreement as the “Parent(s)” irrespective of their actual legal relationship to the Student(s)) of the "Student(s)" identified below (all of whom are collectively referred to in this Agreement as the “Student(s)”) as of the day and year set forth opposite the signature of the last party to sign this Agreement on Page 3.
