This Business Associate Agreement (“BAA”) supplements and is made a part of the agreement by and between the City and County of San Francisco, the Covered Entity (“CE”), and CONTRACTOR/GRANTEE (“Contractor”), the Business Associate (“BA”), dated GRANT START DATE (“Agreement”)]. To the extent that the terms of the Agreement are inconsistent with the terms of this BAA, the terms of this BAA shall control.
