This Master Services Agreement (this “MSA” or “Agreement”) is made and entered by and between Augusta Ada Analytics, LLC, a Delaware limited liability company (“Augusta Analytics”), and Patient/Provider/Authorized HIPAA third-party representative requesting Electronic Health Records through Augusta Analytics (“Customer”) for the treatment and care of its patients. Augusta Analytics and Customer may be referred to herein individually as a “Party” and collectively as the “Parties.” The effective date of this Agreement shall be the date as of the last date fully executed by the Parties (the “Effective Date”).
