This Services Agreement (“Agreement”), effective as of February 28, 2017 (“Effective Date”), is between County of San Bernardino on behalf of Arrowhead Regional Medical Center, located at 400 N. Pepper Ave., Colton, CA 92324 (“Licensee”) and Forward Health Group, Inc., a Delaware Corporation, located at 1 South Pinckney Street, Suite 301, Madison, WI 53703 (“FHG”). FHG and Licensee may be referred to herein individually as a “Party” or collectively as the “Parties.”
