Through its Physician, the Practice delivers Primary Care medicine in a direct pay primary care, membership model (DPC), at 721 E. 14th Street Houston, Texas 77008. In exchange for certain fees, the Practice agrees to provide the Patient with the primary care services described in this Agreement, under the terms and conditions contained within.
