THIS AGREEMENT made and entered into this day of , A.D., 20 by and between Alachua County, a charter county and political subdivision of the State of Florida, by and through its Board of County Commissioners, hereinafter referred to as the "County," and Alachua County Organization for Rural Needs, Inc. dba ACORN Clinic, a not-for-profit corporation organized under the laws of the State of Florida, hereinafter called “Agency.” Collectively hereinafter the County and the Agency are referred to as the “Parties”.
