This Project Plan, effective the 1st day of January, 2018 (“Effective Date”), is by and between Mayo Clinic Arizona, an Arizona charitable corporation, located at 13400 East Shea Boulevard, Scottsdale, AZ 85259 and its affiliates, (collectively "Mayo") and International Clinical Research Centre of St. Anne’s University Hospital in Brno (Fakultni nemocnice u svate Anny v Brne), having a principal place of business at Pekarska 53, 65691 , Brno, Czech Republic ("FNUSA-ICRC"), and is issued pursuant to the Research Collaboration Agreement, effective January 1, 2018 (“Agreement”).
