I/We custodial parent(s) guardian(s), hereby agree to enroll my son/daughter, in the Walpole Public Schools Extended Day Program, subject to the terms in this Agreement. Attendance in this program will be days per week. I/We agree to the monthly tuition obligation of $ (applicable discount rates have been applied). I/We acknowledge that our registration is incomplete without the non-refundable deposit, which consists of a $35.00 registration fee per child and first month's tuition. The deposit is non-refundable without one month's notice.
