This Class Action Settlement Agreement and Release, dated [ ], 2024, is made and entered into by and among Plaintiff, for herself individually and on behalf of the Settlement Class (as defined below), and Defendant Commonwealth Health Physician Network dba Great Valley Cardiology (“GVC”). This Settlement Agreement fully and finally resolves and settles all of Plaintiff’s and the Settlement Class’s Released Claims, upon and subject to the terms and conditions hereof, and subject to the Court’s approval.
