This Agreement (“Agreement”) is effective as of date of last Party’s signature (the “Effective Date”), by and between Board of Trustees of the Leland Stanford Junior University on behalf of the School of Medicine and its Department of Epidemiology and Population Health, a trust with corporate powers under the laws of the State of California and having a place of business at the Alway Building, Suite M121, 300 Pasteur Drive, Stanford, California 94305, and hereinafter referred to as "Stanford" and _________________________, a ___________________having its principal place of business at __________________, hereinafter referred to as "Collaborator." Collaborator and Stanford may be referred to herein collectively as “Parties” or individually as “Party.”
