This Agreement (“Agreement”) is made by and between The School Board of Orange County, Florida, a political subdivision of the State of Florida, located at 445 West Amelia Street, Orlando, FL 32801, (hereinafter referred to interchangeably as “Orange County Public Schools”, “OCPS”, “Board”, “School Board”, or “District”), and Hazel Health, Inc., whose business address is 118 Second Street, Floor 6, San Francisco, CA 94105 (hereinafter referred to as “Contractor”). Each entity shall be referred to individually as a “Party” and collectively as, the “Parties.”
