This CONSULTING SERVICES AGREEMENT ("Agreement") is made and entered into as of the (Date), ("Effective Date"), by and between the FIVE CITIES FIRE AUTHORITY (“AUTHORITY”), a multi-jurisdictional agency created through a Joint Powers Agreement with a place of business at 140 Traffic Way, Arroyo Grande, CA 93420 and (Name of Consultant), a private corporation (“CONSULTANT") with a principal place of business at (address of contractor).
