I, am hereby contracting with the City of Lakeway Activity Center to instruct at (location) on the following day(s) and times of the week: NOTE: The center will be closed on city holidays—New Years’ Day, MLK Day, Presidents’ Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Friday after Thanksgiving, Christmas (2 days as scheduled). Please adjust schedule accordingly.
