This Business Associate Agreement (“BAA”) is entered into by and between the Ventura County Medi-Cal Managed Care Commission, a California local public agency, doing business as Gold Coast Health Plan (“GCHP”) , and _________________________, a _________________ (“Business Associate”), effective ______________ (“Effective Date”). GCHP and Business Associate are each a “Party” to this BAA and are collectively referred to as the “Parties.” Any extensions or renegotiations of this BAA shall be reviewed by both Parties.
