This Voluntary Separation Plan Agreement and Release and Waiver of Claims (“Agreement”) is entered into between [NAME] (“Employee”) and The Board of Trustees of the University of Illinois (the “University”) (collectively the “parties”), pursuant to the terms of the University of Illinois at Urbana-Champaign 2021 Division of Intercollegiate Athletics Voluntary Separation Plan (the “DIA-VSP”). This Agreement shall become effective on the Effective Date as defined in Section 8 below.
