This Agreement is made and entered into by and between the County of Butte, a political subdivision of the State of California, through its Butte County Department of Behavioral Health hereinafter referred to as “COUNTY”, and INSERT CONTRACTOR NAME/COMPANY, hereinafter referred to as “CONTRACTOR”. COUNTY and CONTRACTOR are hereinafter collectively referred to as “PARTIES” or individually “PARTY”.
