Supplemental Agreement entered into by and between the insert contracting entity name, hereinafter called the Owner and insert company name of Architect or Engineer, hereinafter called the Consultant. (The term "Consultant" means the Architect or Engineer individual or firm acting as the Professional-of-Record.)
