The Department of State Health Services (“DSHS” or “System Agency”), an administrative agency within the executive department of the State of Texas and having its principal office in Austin, Texas, and GPCH, LLC, DBA Golden Plains Community Hospital (“Contractor”), having its principal office in Borger, TX (each a “Party” and collectively “the Parties”), enter into the following agreement (the “Contract”) for Tuberculosis Prevention and Elimination (“TB”) services for Public Health Region 1 under Open Enrollment No. HHS0011974.
