The Participant, by signing the State Farm Mutual Funds SIMPLE IRA Application (the “Application”), and State Farm Bank®, Bloomington, Illinois (hereinafter referred to as “Custodian”) by processing the Application and opening the Account have created this Custodial Account Agreement (the “Agreement”) to establish a SIMPLE Individual Retirement Account (“SIMPLE IRA”) under Internal Revenue Code Section 408(p). A contribution has been made on behalf of the Participant to the SIMPLE IRA as indicated on the Application. The Application is hereby made a part of this Agreement.
