This Provider Agreement (hereinafter "Agreement") is made and entered into by and between Rocky Mountain Hospital and Medical Service, Inc., doing business as Anthem Blue Cross and Blue Shield and HMO Colorado, Inc. doing business as HMO Colorado (hereinafter "Anthem") and County of Ouray dba Ouray County Ambulance (hereinafter "Provider").
