This ESO Subscription Agreement (this “Agreement”) is entered into as of _ (“Effective Date”), by and between ESO Solutions, Inc., a Texas corporation having its principal place of business at 11500 Alterra Parkway, Suite 100 Austin, TX 78758, including its controlled subsidiaries, (collectively, “ESO”) and Levy County Public Safety (“Customer”)., having its principal place of business at 1251 NE County Road 343, Bronson, Florida 32621 This Agreement consists of these terms and conditions (the “General Terms & Conditions”) below, the Business Associate Agreement, and any Order (as defined below) executed by the parties, including any attachments to such Order.
