This BUSINESS ASSOCIATE AGREEMENT (this “Agreement”) is entered into by and between (“Pharmacy”) and The Alliance for Patient Medication Safety (“Business Associate”). Pharmacy and Business Associate may each be referred to herein as a “Party” and collectively as the “Parties.”
