By signing this I Parent/Guardian of agree to make payments on or by the specified dates at the agreed amounts stated below on the payment schedule to Ankeny Cheerleading. I undersand the consequences that will be brought against me if the contract is voided. The penalties could include: Account being turned over to collections, dismissal from the Ankeny Cheerleaders, and/or prosecution in small claims court. Upon default, I agree to pay any fees and costs that the Ankeny Cheerleaders or Ankeny High School may incur in collecting my balance owed as well as any late fees/costs if my payments are more than 1 week late.
