The undersigned hereby makes application for space in the Lakeview Band Parents Annual Christmas Fantasyland Craft Show. The fee for a 10x6 space(s) shall be $80.00 for 1 space, $150.00 for 2 spaces, and $215.00 for 3 spaces. This is the total price for the 2-day show, not a per day price. Any Lakeview Public Schools sponsored groups will be charged $25.00 for the first year of participation and above mentioned participant price for any year thereafter. There is an early bird discount and a late entry fee (see #10 below). This sum shall be due at the time of application for space to be reserved and is payable by check or money order made out to the Lakeview Band Parents Association.
