THIS TERMS OF SERVICE AGREEMENT (together with all Exhibits, this “Agreement”), made this March 10, 2025[enter appropriate future da], (the “Effective Date”) is entered into by VaxCare, LLC, a Florida limited liability company located at 3113 Lawton Road, Suite 250, Orlando, Florida 32803 (“VaxCare”) and the City of Nashua, New Hampshire, a municipal corporation located, for this purpose at the City of Nashua Division of Public Health and Community Services, 18 Mulberry Street, Nashua, New Hampshire, 03060 (“Administrator”).
