This Vendor Agreement (Agreement) is entered between the State of Florida, Department of Health (the Department) and INSERT VENDOR NAME (Vendor), together the “Parties”, pursuant to Vendor’s application to the Department requesting participation in the Special Supplemental Nutrition Program for Women Infants and Children, authorized by Section 17 of the Child Nutrition Act of 1966, as amended (the WIC Program).
