This CONTRACT is made and entered into between, John Leidy, EA on behalf of JDBC Tax Services (collectively the "Accountant"), federally designated as an Enrolled Agent to practice as a tax and accounting professional, and having a principal place of business at 5423 West Vliet Street, Milwaukee, Wisconsin 53208, and the “Client” listed on the Tax and Bookkeeping Agreement effective the date listed on the Agreement.
