This agreement (“Agreement”) is entered into (“Effective Date”) by and between _________________ (“Contractor”) and Tarrant County Hospital District d/b/a JPS Health Network (“Customer”) according to the following terms and conditions. Contractor agrees to provide and deliver to Customer and Customer agrees to purchase the Services (defined below) for the price and according to the terms and conditions set forth in this Agreement. In this Agreement, each of Contractor and Customer are a “party” and both of them collectively are the “parties”.
