By signing below, I express my desire and commitment to participate in the cargo securement program (the “Program”) available from Cargo Securement Academy, Inc. (“CSA”) for commercial truck drivers. I understand that participation in the Program will provide me with training in the proper securement of cargo for over-the-road transportation on commercial motor vehicles.
