This Amendment (“Amendment No. 3”) to Contract No. MA-042-19010157 for Non- Emergency Transportation Services entered into this 1st of July 2022 (“Effective Date”) is by and between CABCO YELLOW, INC., DBA CALIFORNIA YELLOW CAB (“Contractor”) a California for profit corporation, with a place of business at 520 W. Dyer Road, Santa Ana, CA 92707, and the COUNTY OF ORANGE, a political subdivision of State of California (“County”), through its Health Care Agency, with a place of business at 405 W. 5th St., Ste. 600, Santa Ana, CA 92701. County and Contractor may sometimes be referred to herein individually as “Party” or collectively as “Parties.”
