This EpicCare Link Access Agreement (this “Agreement”), effective as of (the “Effective Date”), is entered into between Oakleaf CIN, LLC (“CIN”) and (“Outside Entity”), a licensed health care provider and/or a Business Associate of CIN bound by the terms of a separate Business Associate Agreement. CIN and Outside Entity are sometimes referred to in this Agreement individually as a “Party” and, collectively, as the “Parties”.
