INFECTION SCREENING CONTRACT (the “Contract”) is entered into by and between the Arizona Family Health Partnership dba Affirm Sexual and Reproductive Health, an Arizona not-for-profit corporation (“Affirm”), and Pinal County Public Health Services District (the “Contractor”). Affirm or the Contractor may be referred to individually as the “Party” or collectively the “Parties”.
