This Agreement (“Agreement”) is between the General Agent (“GA”) listed on the Contract Information and Signature Form and Mutual of Omaha Medicare Advantage Company (“MOMAC”), Omaha Health Insurance Company (“OHIC”), and Medicare Advantage Insurance Company of Omaha (“MAICO”) (together MOMAC, OHIC, and MAICO are referred to as “Company”) and is effective on date this Agreement is executed by Company as indicated on the execution page (the “Effective Date”).
