This Settlement Agreement is made by and between plaintiffs Mary E. Ormond and Kevin T. Heekin, on behalf of themselves and the Class (as hereinafter defined), on the one hand, and defendants Anthem Insurance Companies, Inc. and Anthem, Inc., n/k/a WellPoint, Inc., on the other hand. This Agreement fully and finally compromises and settles any and all claims that were or could have been asserted in the lawsuit styled as Mary E. Ormond, et al., v. Anthem, Inc., et al., Case No. 1:05-cv-01908-TWP-TAB (S.D. Ind.).
