This Participant Agreement ("Agreement") is entered into effective 1st day January 1, 2020 (“Effective Date”), by and between Press Ganey Associates, Inc. (“Press Ganey”), an Indiana corporation with principal offices at 404 Columbia Place, South Bend, Indiana 46601, and The County of Monterey, a political subdivision of the State of California (hereafter, "County of Monterey on behalf of Natividad Medical Center ("Participant") physically located at 1441 Constitution Boulevard, Salinas, CA 93906.
