The undersigned has voluntarily elected to use and, if applicable, to allow the minor child(ren) identified below and all minor children under my supervision and referred to individually and collectively herein as “Child,” to use the Whitaker Center facilities and equipment located at 222 Market Street, Harrisburg, PA 17101. In consideration for being allowed to use said facilities and equipment, and any other services provided by Whitaker Center or its employees or agents at said location, or any other location within the Commonwealth of Pennsylvania, I represent, acknowledge and agree as follows:
