The Health and Human Services Commission (“HHSC” or “System Agency”) and Easter Seal Rehabilitation Center, Inc. (“Grantee”), who are collectively referred to as the “Parties,” to that certain Early Childhood Intervention Services Grant Program Contract that was effective September 1, 2020 and denominated as HHSC Contract No. HHS000640200029 (the “Contract”), now desire to amend the Contract.
