THIS SERVICES AGREEMENT (this “Agreement”) is made and entered into on the 1st day of August, 2019 (“Commencement Date”) by and between Fairview Health Services (“Fairview”), a Minnesota nonprofit corporation, and Independent School District No. 912 (“District”), contracting on behalf of Milaca High School (“School”) for a two-year period, the 2019-2020 and 2020-2021 school years (the “Term”). Fairview, District, and School may be referred to throughout this Agreement in singular as “party” and collectively as “parties”.
