HODNOCENÍ

Druhý dodatek („Dodatek”) ke Smlouvě o klinickém hodnocení, uzavřené s účinností od 18. května 2023, pozměněné Dodatkem č. 1 ze dne 14. listopadu 2023 (dále hromadně „Smlouva“) mezi Fakultní nemocnici v Motole, V Úvalu 84, 150 06 Praha 5, Česká

republika, IČ: 000 64 203, DIČ: CZ00064203,

zastoupená , na

základě pověření („Poskytovatel zdravotních služeb“ nebo jen „Poskytovatel“) ve spolupráci s

(„Hlavní zkoušející”) a AbbVie, s. r. o., Metronom Business Center, Bucharova 2817/13, ▇▇▇▇▇▇▇▇, ▇▇▇ ▇▇ ▇▇▇▇▇ ▇, ▇▇▇▇▇ ▇▇▇▇▇▇▇▇▇, IČ: 24148725, DIČ: CZ24148725, zastoupené Country Clinical Operations Manager, na základě plné moci („AbbVie”) na poskytování služeb uvedených v Protokolu č. M20- 638 s názvem „Otevřená studie fáze 3 hodnotící bezpečnost a účinnost epcoritamabu v kombinaci s rituximabem a lenalidomidem (R2) v porovnání s R2 u pacientů s relabujícím nebo refrakterním folikulárním lymfomem (EPCORETM FL-1)“.

S ohledem na řádné plnění tohoto Dodatku a v souladu s podmínkami ▇▇▇▇▇▇▇, se smluvní strany dohodly pozměnit Smlouvu následovně:

1. Rozpočet stanovený v Příloze A Smlouvy se zrušuje v celém svém rozsahu a nahrazuje se novým zněním Přílohy A, která tvoří přílohu tohoto dodatku.

Pojmy, jež zde nejsou definovány jinak, mají stejný význam, který je jim připisován ve ▇▇▇▇▇▇▇. Kromě podmínek, které se výslovně změnily tímto Dodatkem, zůstávají všechny ostatní podmínky této Smlouvy v plné platnosti a účinnosti během trvání této Smlouvy.

Účinnost tohoto dodatku nastává dnem uveřejnění v registru smluv a Smluvní strany souhlasí, že výše plateb za činnosti uvedené v Příloze A budou kalkulovány i pro proběhlé procedury ode dne

upon delegation of powers (“Health Care Provider” or only “Provider”) and

Country Clinical Operations Manager, upon basis of power of attorney (“AbbVie”) for services relating to Protocol No. M20-638 entitled “A Phase 3, Multicenter, Randomized, Open-Label Trial of Epcoritamab in Combination with Rituximab and Lenalidomide (R2) compared to Rituximab and Lenalidomide (R2) alone in Subjects with Relapsed or Refractory Follicular Lymphoma(EPCORETM FL-1).”

Subject to the full execution of this Amendment and in accordance with the terms of the Agreement, the parties hereby agree to amend the Agreement, as follows:

1. The Budget set forth in Exhibit A of the Agreement shall be deleted in its entirety and replaced with the attached Exhibit A, as integral part of this Amendment.

Terms not otherwise defined herein shall have the meanings ascribed to such terms in the Agreement. Except as specifically amended by this Amendment, all other terms and conditions of the Agreement shall continue in full force and effect during the term of the Agreement.

This Amendment shall become effective as of the date of its publication in Contracts Registry and the Parties agree that the amount of payments for the

procedures listed in the Exhibit A will also be calculated for procedures performed from 22
