Class Definition. You are a member of the “Settlement Class” if you purchased or otherwise acquired Vivint Solar common stock between March 5, 2019 and September 26, 2019 (the “Class Period”) and suffered alleged damages as described in the Notice, and do not “opt out” of the Settlement Class. Settlement Amount: $1,250,000. This represents an estimated average recovery of $0.10 per allegedly damaged share of Vivint Solar common stock, before attorneys’ fees and expenses, based on the estimated number of allegedly damaged shares of Vivint Solar common stock acquired during and held through the end of the Class Period.

Class Definition or "Class as defined" shall mean and refer to the class and subclasses defined by the Court by order dated November 10, 1997. By that order, the Court defined the class as follows: All persons and legal entities located or residing in, owning places of business or property in, operating businesses in, attending school or working in, and/or who were present within the geographic area specified herein, who sustained bodily and/or personal injury, loss, property damage, and/or other damage, as the result of an explosion, rupture, escape and/or leak or leaks of nitrogen tetroxide (N204) and their derivatives from a railroad tank car, mobile truck tanker-trailers, and/or related loading/unloading piping or other structures located on the premises of Gayl▇▇▇ ▇▇▇mical Corporation in Bogalusa, Louisiana, occurring in and during October 1995, as well as those family members of such persons who may themselves have claims arising out of injury to such persons. The geographic boundaries of the class are defined below. The definition includes a reference to the "City Rectangle" which will be defined as the area within the smallest rectangle which will encompass all of the City Limits of Bogalusa. The class boundaries will be the area within the following truncated wedge-shaped region: 1) begin at the southeast corner of the City Rectangle, and extend the line comprising the southern side of the City Rectangle until the point at which it reaches the Pearl River; 2) then follow the western bank of the Pearl River northward until reaching the northeastern most point of the boot of Louisiana; 3) from that point, draw a straight line to the confluence of Gully Creek, Upper Little Creek, and Kellers Creek in Mari▇▇ County, Mississippi; 4) from the confluence of those three creeks in Mari▇▇ ▇▇▇nty, Mississippi, draw a straight line in a westerly direction to the intersection of State Highway 570 and U.S. Highway 51 in Pike County, Mississippi; 5) then from the intersection of State Highway 570 and U.S. Highway 51 draw a straight line to the southwestern corner of the City Rectangle, in Washington Parish, Louisiana; 6) continue east along the southern boundary of the City Rectangle until reaching the point of beginning at the southeastern corner of the City Rectangle.

Class Definition. “All individuals who scanned their finger in Elgin Die’s timekeeping system in Illinois between January 15, 2018 to March 19, 2024 without first providing written consent.” Time to Effectuate Notice: 30 days after Preliminary Approval Time to Submit a Claim Form: 90 days after Preliminary Approval Time to Object or File Exclusion: 90 days after Preliminary Approval Time for Final Hearing Date: 120 days after Preliminary Approval, or as otherwise set by the Court.

Class Definition or “Class as Defined” shall mean and refer to the following: A. Any and all Providers in Louisiana from January 1, 2000 through January 11, 2007, including, without limitation, those Providers more fully described in Subsections B and C below. B. Any of the Providers included in Subsection A above that had one or more bills for medical services discounted through any Preferred Provider Organization contract, agreement, or understanding with Focus and/or with any entity that contracted with Focus, either directly or through one or more intermediaries, to offer preferred provider discounts to Focus and/or Focus’s client(s). C. Any of the Providers included in Subsection A above that had one or more bills for medical services discounted through any Preferred Provider Organization contract, agreement, or understanding with Concentra and/or with any entity that contracted with Concentra, either directly or through one or more intermediaries, to offer preferred provider discounts to Concentra and/or Concentra’s client(s). D. Anything to the contrary contained herein notwithstanding, Focus and Concentra are excluded from the class. Capitalized terms as used herein shall have the following definitions:

Class Definition. “All individuals who used Defendant’s finger scan timekeeping system in Illinois between March 13, 2020 to July 31, 2023, and had their finger scanned and/or biometric identifier, biometric information or other biometric data collected, captured, received, converted, stored, used, disclosed or disseminated, without first executing a written consent prior to the initial scan, collection, capture, receipt, disclosure or dissemination of their biometric data.” Time to Effectuate Notice: 30 days after entry of the Preliminary Approval Order. Time to Object or File Exclusion: 90 days after entry of the Preliminary Approval Order. Time for Final Hearing Date: 120 days from after entry of the Preliminary Approval

Class Definition or “Class as Defined” shall mean and refer to the following: “All medical providers, institutions, and facilities that have provided services to workers’ compensation patients pursuant to the Louisiana Workers’ Compensation Act, LSA-R.S. 23:1021 et seq., and whose bills have been discounted, adjusted, paid on a reduced basis, or otherwise paid at less than the billed amount pursuant to a Preferred Provider Agreement contracted with CorVel or owned or operated by CorVel. “ In the event the Court should alter or modify the above class definition, and such amended class definition is accepted in writing by the PSC and CorVel, such amended class definition shall be considered the “Class Definition” or “Class as Defined” under this Settlement Agreement, and all references to “Class Definition” or “Class as Defined” in this Settlement Agreement shall mean and refer to such accepted amended class definition.

Class Definition or “Class as Defined” shall mean and refer to the following: All Louisiana healthcare providers who, on or after the date of the filing of the instant petition [August 24, 2016], 1) are citizens of the state of Louisiana, and 2) are contracted with Defendant Louisiana Blue Cross [BCBSLA], and 3) have had one or more medical bills reimbursed pursuant to such contract for any services rendered to a patient in the state of Louisiana. In the event the Court should alter or modify the above class definition, and such amended class definition is accepted in writing by the PSC and BCBSLA, such amended class definition shall be considered the “Class Definition” or “Class as Defined” under this Settlement Agreement, and all references to “Class Definition” or “Class as Defined” in this Settlement Agreement shall mean and refer to such accepted amended class definition.
