Code 4. Has the meaning set forth in Section 4.2(d) of Schedule B to the Terms and Conditions Schedule B
Code 4. Code 4 has been assigned to Service Levels that the Parties agree do not require validation or baselining. Service Provider shall meet or exceed Code 4 Service Levels beginning on the Service Commencement Date.

  • The State reserves the right to withhold payment of the Engineer's billing statement in the event of any of the following: (1) If a dispute over the work or costs thereof is not resolved within a thirty day period; (2) pending verification of satisfactory work performed; (3) the Engineer becomes a delinquent obligor as set forth in Section 231.006 of the Family Code; (4) required reports are not received; or (5) the State Comptroller of Public Accounts will not issue a warrant to the Engineer.

Code 4. For a Code 4 Issue which is not resolved within one hundred and twenty-five (125%) of the Response Commitments and where such Code 4 Issue has a “material impact” on Customer such as a consistent, repeated calculation error that results in payment inaccuracy, Customer shall receive five (5) Performance Credits for each seven (7) business day period beyond the Response Commitments up to a total of fifteen (15) Performance Credits for each Code 4 Issue.

OHI and appropriate therapy, including subgingival plaque removal, plus removal of calculus, correction of plaque-retentive margins of restorations, root planing as indicated, extraction, or other therapy as deemed appropriate by the treating dentist. The periodontal charting should be completed prior to initiation of nonurgent/emergent dental care.
