Substitute form means a form that is intended to replace a department form. This rule is intended to implement Iowa Code sections 17A.3(1)“b” and 421.14.

  • In order to facilitate the investment of cash Collateral on behalf of the Funds, and as a condition precedent to the effectiveness of this Agreement, the Client shall, at the request of the Lending Agent, execute on behalf of the Funds and deliver to the Lending Agent, a Subscription Agreement and/or Substitute form W-9 in such form as may be prescribed by the applicable investment fund from time to time.
