Transition Coordinators definition

Transition Coordinators shall have the meaning specified in Section 7.8.
Transition Coordinators has the meaning assigned to such term in Section 3.5.
Transition Coordinators has the meaning set forth in Section 6.9(c).

Examples of Transition Coordinators in a sentence

  • The Transition Coordinators shall meet at least monthly in person and shall meet together quarterly with the Chief Executive Officers of Parent and the Company.

  • Review individual files and other data the State and its contracted entities maintain and interview Target Population members, Providers, Case Managers, Transition Coordinators, In-Reach Counselors, and relevant State staff.

  • Any dispute that is not resolved by the Transition Coordinators within forty-five (45) calendar days shall be resolved in accordance with the dispute resolution and arbitration procedures set forth in Section 11.8.   • Phase 2 will include programming of the Transition Assessment, Transition Plan, and post- transition monitoring efforts by the Transition Coordinators into the system.

  • The MCOs will provide MFP Transition Coordinators and Nurses that will develop transition plans and assist MFP eligible clients in transitioning from institutions to the facility.

  • The State will provide regular training to Case Managers, Transition Coordinators, nursing facility staff, and other relevant Providers to ensure these individuals are knowledgeable of the Home Maintenance Allowance program and its role in reintegrating At-Risk Population members to the Community.

  • Such meetings shall be held at such times as is necessary to accomplish the purposes set forth in the foregoing sentence; either Transition Coordinator may by written or E-mail communication to the other Transition Coordinator request that a meeting be scheduled and the Transition Coordinators shall then cooperate in good faith to schedule the requested meeting.

  • The Service Provider and the Service Recipient shall advise each other promptly (in any case within no more than three (3) Business Days) in writing of any change in their respective Transition Coordinators, setting forth the name of the replacement, and stating that the replacement Transition Coordinator is authorized to act for such Party in accordance with this Section2.5(c).

Related to Transition Coordinators

  • Services Coordinator means an employee of the community developmental disability program or other agency that contracts with the county or Department, who is selected to plan, procure, coordinate, and monitor Individual Support Plan services, and to act as a proponent for individuals with developmental disabilities.

  • Service Coordinator has the meaning set forth in Section 2.2.

  • Program coordinator means a registered nurse responsible for administrative aspects of a state-approved nurse aide training course.

  • Project Coordinator means the employee of Alamo Colleges District designated in Exhibit A hereto who will manage the relationship between Alamo Colleges District and Contractor. The designated employee will be knowledgeable of the Project and be experienced in managing projects similar to the one established herein.

  • Scheduling Coordinator or “SC” means an entity certified by the CAISO as qualifying as a Scheduling Coordinator pursuant to the CAISO Tariff for the purposes of undertaking the functions specified in “Responsibilities of a Scheduling Coordinator,” of the CAISO Tariff, as amended from time to time.
