Access to Employee Files. 13.01 YSB shall provide adequate and continuous supervision for Employees throughout their term of employment. Upon request, Employees shall have access to their supervision file. Supervision notes will be documented in a timely fashion. Each Employee shall attend all designated periods of instruction in supervision held during the Employee's working hours, and be prepared as required for same. Supervision will be scheduled at a mutually agreeable time except in cases of emergency. 13.02 Upon request, an employee has the rights to review information contained within their personnel file in the presence of the Executive Director or her designate. The employee has the right to make copies of any information they have access to. The employee has the right to respond in writing to any such document which is placed within their personnel file within fifteen (15) work days. Such reply shall become part of the permanent record. 13.03 Any verbal reprimand, letter of reprimand, suspension, termination or other disciplinary sanction, with the exception of those which concern professional misconduct regarding clients, shall be removed from the file of an Employee eighteen (18) months following the receipt of such letter, suspension or other sanction, provided that the Employee's record has been discipline free for such eighteen (18) month period.
