Common use of Accident Reporting Clause in Contracts

Accident Reporting. If a death, serious personal injury, or substantial property damage occurs in connection with the performance of this Agreement, Contractor shall immediately notify the Senior Manager of Business Services, by mail, at the following address: Judicial Council, Administrative Office of the Courts, Senior Manager, Business Services, ▇▇▇ ▇▇▇▇▇▇ ▇▇▇▇ ▇▇▇., ▇▇▇ ▇▇▇▇▇, ▇▇▇ ▇▇▇▇▇▇▇▇▇, ▇▇ ▇▇▇▇▇-▇▇▇▇. Contractor shall promptly submit a written report, in such form as may be required by the AOC, of all accidents, which occur in connection with this Agreement. The report must include at least the following information: Name and address of the injured or deceased person(s); Name and address of Contractor’s subcontractor, if any; Name and address of Contractor’s liability insurance carrier; A description of the circumstances surrounding the accident, whether any of the AOC’s equipment, materials or staff were involved and the extent of damage to AOC and/or other property; and A description of what effect, if any, the accident will have upon Contractor’s ability to perform the Work.
