Common use of Applying for Leave Clause in Contracts

Applying for Leave. a.) An employee’s application for a leave of absence must be made through the designated third party administrator (TPA) thirty (30) calendar days in advance of the leave, except in cases of emergency. b.) In cases of emergency, the employee shall contact his/her manager and explain the circumstances requiring emergency leave and must notify the TPA and submit an application for leave, as soon as possible, but not later than seven (7) consecutive scheduled work days after being absent from work. The leave will either be approved or not approved; the employee must complete the appropriate paperwork within seventy-two (72) hours of the time the leave is approved. c.) The employee’s application must include the beginning and end dates of the leave, with statement of the employee’s intent to return to work.
