BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The CONSULTANT will complete the Additional Services to the City’s full satisfaction and in accordance with Exhibit A of this Supplemental Agreement No. supplemental agreement number.

BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The CONSULTING ENGINEER/ARCHITECT will complete design services in accordance with Exhibit A of this Supplemental Agreement in order to provide preliminary work for utilities and additional design for retaining walls and driveways for the 2009 Residential Street portion of the contract, and additional work for North Park detention and additional storm sewer design and public relations for the SMAC portion of the contract.

BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The CONSULTING ENGINEER/ARCHITECT will complete additional design services in accordance with Exhibit A of this Supplemental Agreement No. 3 for College Boulevard Widening (▇▇▇▇▇▇ Road to U.S. 69 Highway). The CONSULTING ENGINEER/ARCHITECT will complete the services as follows: All work shown in Exhibit A of this Supplemental Agreement No. 3 shall be completed within 30 calendar days from the date of execution of this Supplemental Agreement No. 3.

BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The Consulting Engineer/Architect will complete the Additional Services to the City’s full satisfaction and in accordance with Exhibit A of this Supplemental Agreement No. 2. All work shown in Exhibit A of this Supplemental Agreement No. 2 shall be completed in accordance with the Project time frame set forth in the Original Agreement except as specifically modified by Supplemental Agreement No. 1 dated January 13, 2014. The Additional Services will be provided at an amount not to exceed Fifty Thousand Six Hundred Sixty and no/100 Dollars ($50,660.00) including reimbursables in accordance with Exhibit B, attached hereto and incorporated by reference herein. This Supplemental Agreement No. 2 raises the maximum fee to Five Hundred Sixty Five Thousand Five Hundred Ninety and no/100 Dollars ($565,590.00) for the Project. This is the total of the fee from the Original Agreement of One Hundred Forty Five Thousand Six Hundred Fifteen and no/100 Dollars ($145,615.00) plus Three Hundred Sixty Nine Thousand Three Hundred Fifteen and no/100 Dollars ($369,315.00) for Supplemental Agreement No. 1 plus Fifty Thousand Six Hundred Sixty and no/100 Dollars ($50,660.00) for Supplemental Agreement No. 2.

BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The CONSULTING ENGINEER/ARCHITECT will complete design services in accordance with Exhibit A of this Supplemental Agreement for Final Design, Bidding, and Construction Services. The CONSULTING ENGINEER/ARCHITECT will complete the services as follows: All work shown in Exhibit A of this Supplemental Agreement shall be completed on or before November 1, 2009. The services listed in Exhibit A of this Supplemental Agreement No. 1 will be provided at an amount not to exceed One Million Seven Hundred Twenty Five Thousand Dollars ($1,725,000) This Supplemental Agreement raises the maximum fee to One Million Seven Hundred Seventy Four Thousand Three Hundred Dollars ($1,774,300) for the project. This is the total of the original fee of Forty Nine Thousand Three Hundred Dollars ($49,300) plus One Million Seven Hundred Twenty Five Thousand Dollars ($1,725,000) for Supplemental Agreement No. 1. Payment to Consulting Engineer/Architect shall not exceed the following percentages in each phase of the Project without prior written consent of City: Phase of Work Fee Percentage · Design Development 25% · Construction Documents 38% · Bidding and Negotiation 5% · Construction Phase 27% · Project Closeout 4.5% · 10 month Warrantee review 0.5% TOTAL 100% Fee percentages identified will be billed relative to the percentage complete of each construction document bid package.

BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The CONSULTING ENGINEER/ARCHITECT will complete design services for architectural, structural engineering and electrical engineering services for the 30 foot addition to the ▇▇▇▇▇▇ ▇. ▇▇▇▇▇▇▇ ▇▇▇▇ ▇▇▇▇. The CONSULTING ENGINEER/ARCHITECT will complete the services as follows: All additional design work shall be completed by January 15, 2009.

BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The CONSULTANT will complete the Additional Services to the City’s full satisfaction and in accordance with Exhibit A of this Supplemental Agreement No. 3. The CONSULTANT will complete the Additional Services in the time frame set forth below. All work shown in Exhibit A of this Supplemental Agreement No. 3 shall be completed by The Additional Services listed in Exhibit A of this Supplemental Agreement No. 3 will be provided at an amount not to exceed one hundred one thousand six hundred seventy-five dollars ($101,675.00) including reimbursables. This Supplemental Agreement raises the maximum fee to one million eight hundred two thousand nineteen and 50/100 dollars ($1,802,019.50) for the project. This is the total of the fee from the Original Agreement of $1,470,485.00 plus $30,000 for contingency plus $148,802.00 for Supplemental Agreement No. 1 plus $51,057.50 for Supplemental Agreement No. 2 plus $101,675.00 for Supplemental Agreement No. 3.

BASIC CONSULTING ENGINEERING/ARCHITECT SERVICES. The CONSULTING ENGINEER/ARCHITECT will complete design services in accordance to Exhibit A of this Supplemental Agreement for the ▇▇▇▇▇ ▇▇▇▇▇▇ ▇▇▇ ▇▇▇▇▇ ▇▇▇▇▇▇ Roundabout Improvements. The CONSULTING ENGINEER/ARCHITECT will complete the services as follows: 1. Engineering and landscaping design was completed to rebid the project with an asphalt alternate and landscape plan modifications. 46 of 75 plan sheets required design modifications. Additionally, quantity calculations were completed to revise the engineer’s construction cost estimate. Plans were distributed to Kansas Department of Transportation (KDOT) for authorization to bid. Bid documents were revised for the new letting.
