Billing Authorization. A Customer representative with signature authority must be available to accept the satisfactory completion of work performed and to authorize billing for each Service event, including Service events provided outside of Normal Working Hours.

Billing Authorization. Responsibility for payment of charges may be handled in one of two ways: 1. The handicapped person (the applicant) may accept responsibility for payment of his or her own bill. In this case, the applicant must be 18 years of age or older and must reside within the Company's service territory, but he or she does not need to have other service from the Company. 2. Another party may agree to accept responsibility for payment of charges incurred through use of the Special Credit Card by the applicant. When this option is chosen, the person accepting this responsibility must be 18 years of age or older, but does not need to reside within the Company's service territory. In either case, the applicant is the only authorized user of the Special Credit Card. If the person accepting payment responsibility has service within the Company's service territory, charges will be billed on a regular monthly bill; otherwise a separate bill will be sent.

Billing Authorization o By subscribing to or using the Services, you expressly agree that YBA is permitted to bill you charges associated with the Price Plan and Extras, any applicable taxes, customs duties, and any other charges you may incur in connection with your use of the Services.

THE SUBSCRIPTION FEE IS EXCLUSIVE OF ANY SUCH TAXES. THE SUBSCRIPTION FEE IS PAYABLE IN U.S. DOLLARS. SUBSCRIPTION CHARGES WILL BE DISPLAYED TO YOU AT THE TIME OF PURCHASE. IF YOU SUBSCRIBE THROUGH A PARTY OTHER THAN CURRENT (E.G., APPLE OR GOOGLE), THEN YOUR INFORMATION WILL BE HANDLED IN ACCORDANCE WITH SUCH PARTY’S RESPECTIVE TERMS OF USE AND PRIVACY POLICIES AND YOU ARE SOLELY RESPONSIBLE FOR ENSURING YOUR ACCEPTANCE OF SUCH PARTY’S TERMS OF USE AND PRIVACY POLICIES. NOTHING HEREIN OBLIGATES CURRENT TO OFFER A SUBSCRIPTION OPTION THROUGH THE WEBSITE OR ANY APP.

Network Authorization For services that cannot be provided by a network provider, you can request a network authorization to seek services from a non-network provider. With an approved network authorization, the network benefit level will apply to the authorized covered healthcare service. If we approve a network authorization for you to receive services from a non- network provider, our reimbursement will be based on the lesser of our allowance, the non-network provider’s charge, or the benefit limit. LEGAL AUTHORIZATION The Recipient certifies that it has the legal authority to receive the funds under this Agreement and that its governing body has authorized the execution and acceptance of this Agreement. The Recipient also certifies that the undersigned person has the authority to legally execute and bind Recipient to the terms of this Agreement.
