Completion of Evaluation Cycle. a. The evaluation shall acknowledge the performance strengths of the teacher evaluated as well as performance deficiencies. The evaluator shall note the data used to support the conclusions reached in the evaluation written report, including pieces of evidence that illustrate specific practices in the identified focus area(s). The evaluation written report shall be signed by the evaluator and the teacher. The teacher’s signature shall serve as acknowledgement that the evaluation written report will be placed in the teacher’s personnel file.
